Twelve columns divide by two, three, four and six. That is why everyone keeps coming back to it.

The note
Why twelve
Twelve is divisible by 2, 3, 4 and 6, so a single grid supports halves, thirds, quarters and sixths. That gives you a page with variety and no arbitrary measurements.
Setting it up
Choose the gutter first, then the margins, then the column width falls out. We use 40 pixels on desktop, 32 on tablet, 16 on phone.
Breaking it
Break the grid on purpose, once per page, and everyone will notice.
